You can check your benefits for outpatient mental health services by calling the Customer Service number on the back of your insurance card and asking the following questions:

• Do I have mental health benefits? For in-network and out-of-network providers?

• What is my annual deductible and has any of it been met? Does my deductible apply to mental health benefits?

• What is my co-pay or co-insurance responsibility once my deductible has been met (if applicable)?

• Is there a limit on the number of sessions covered per year?

• Is an authorization or referral from my primary care physician required?

• Is Kayla Brooks, LMFT a covered provider?

Kayla is able to bill your insurance company for those companies she is contracted with and will also bill if you have out-of-network benefits for those companies she is not contracted with at this time.
